Receives referrals for coordination of home care nursing services. Coordinates home care
services by referral or subcontracting depending on the need. Clinically assess
patient/caregiver knowledge of therapies. Identifies appropriate nursing agencies to provide
training support for patients/caregivers.
Homecare and/or home infusion experience. Computer skills. Excellent customer service skills.
Detail oriented. Multi-tasking capabilities. Leadership experience.
Licensed registered nurse in Maryland.
