Neighborhood Health Planís Community Medical Alliance (CMA) Programs are designed for certain
MassHealth patients who qualify and who have chronic illness and other high risk medical and
psychiatric conditions that include advanced HIV/AIDS, severe physical disabilities, mental
retardation, and developmental disabilities. The CMA Programs provide culturally competent
complete health care for selected NHP patients with complex health care needs. NHP currently
has an opening for an HIV/AIDS Nurse Practitioner. This is a home-based position where the NP
sees patients in a variety of settings, as well as provides care management for them. Our NPs
work autonomously, deciding how much time they can spend with their patients.
Provides primary care, care management and overall coordination of the plan of care to a
specified panel of patients in collaboration with primary care physicians.
