The incumbent is responsible for the overall management of the community-based disease
management services delivered by the field operations for the assigned geographic location.
Works closely with call center management, HIS management and other manager throughout the
company to ensure that field operation programs are meeting the expected levels of performance
according to established operation plans. The incumbent must be culturally sensitive to the
local population. Provides support as required to other departments to promote LifeMasters
services and performs special projects and other duties as assigned.
Manages, directs and handles the overall responsibility for the field operations for the
assigned geographic area. Works closely with supervisor to review and address operational
deliverables for the field operations team according to the client contract and ensure
appropriate business plans are developed, communicated and carried out by the staff to meet
expected service goals for the assigned area.
Negotiates and manages contracts with home health agencies to augment company staff with
finder and nurse services for field operations. Ensures contracts receive appropriate legal
review and approval. Monitors and ensures that agency staff is meeting contract expectations
and goals. Ensures appropriate and timely payments are approved and made to agencies according
to company finance policy and procedures.
Provides support and direction regarding operational functions to business improvement teams
and acts as a liaison to implementation of any and all new business initiatives. Promotes an
environment of support and acceptance to business improvement initiatives, supports front line
team members in implementation of new processes, and holds team members accountable to new

Engages workforce to align with LifeMasters business initiatives.
Effectively leads staff team meetings, including meeting planning, agenda preparation and
distribution of minutes/outcomes.
Plans and organizes work plans to assure that plans are executed, deadlines are met on time
and within budget.
Works cooperatively and engages Team Managers in the field and at various LifeMasters call
centers to promote effective communication and continuity of care. Accepts responsibility for
driving performance toward goals and communicates effectively to peers when process gaps are

Represents LifeMasters in Client and Customer Interactions
a. Conducts and facilitates Contract Meetings with client and customer;

Oversees the delivery of service by field staff for the assigned area. Ensures that staff
receives the appropriate level of on boarding, training and ongoing training along with
operational updates on field operations. Ensure that staff is meeting job performance goals
and expectations by working through team managers and directly with direct reports.
Acts as liaison with assigned call center management to ensure that appropriate communication
is flowing between the call center operation and field operations.
Regularly prepares standard business activity reports for supervisor, HIS management as well
as internal and external customers
Must pay strict attention to follow all HIPAA requirements and LifeMasters company policies
regarding the security and privacy of Protected Health Information (PHI) and related internal
and external security measures of the same. Must follow and comply with all company policies
and procedures in the execution of job duties and ensure all staff fully complies as well.
Incumbent is responsible for fostering a collegial and collaborative relationship with other
health care professionals and health care facilities in the area, including physicians,
nurses, hospitals, nursing homes and other health care professionals and facilities.
Maintains personal and professional competencies and participates in activities which
contribute to the ongoing development of self and profession. Accepts responsibility for own
practice and development, and participates in activities that further develop professional
growth. Demonstrates interest in continuing education through participation in committees,
resource activities, etc. offered by the Company or through outside sources. Identifies
learning needs and seeks assistance from more experienced staff members, manager, etc. Sets
realistic goals for own development based on assessment of strengths and learning needs.
Graduate of accredited school of nursing. Must have taken the NCLEX in the U.S.
Current licensure as registered nurse within the state they will be employed with no
restrictions. Bachelors or Masters Degree in Business or Nursing preferred.
10 + years Registered Nurse experience. 8 + years applicable clinical management experience
overseeing a broad team of clinical and non-clinical staff in a community services, home
health care or related work experience environment. Clinical experience in Cardiac and/or
Diabetes preferred. Experience with geriatric, pediatric or indigent care. 35 wpm typing &
solid basic computer skills required (knowledge of how to use drop down menus, a mouse,
maximize and minimize screens, switch between open windows and programs, tab functions, and
cut and paste feature). Bi-lingual preferred, language may vary by location.
